Patient Forms and Appointments

In order for Connecticut Foot & Ankle Associates to provide the most efficient and comprehensive care to our patients, listed below is the information you will need for an appointment, our office hours, patient pre-operative and post-operative instructions and the healthcare plans and insurance accepted by this practice.
We ask that you download the Patient Registration Form and bring it to your appointment along with the following information:

  • Photo ID or driver’s license
  • Referral from your primary care physician, if applicable
  • Any reports, x-rays or lab results regarding the condition for the scheduled appointment
  • Your current insurance card(s)
  • Your co-payment as indicated in your insurance contract
  • Full payment for services for those patients without insurance
  • We accept cash, checks, Visa/MasterCard

Cancellations – please notify as soon as possible if you need to change or cancel your appointment by calling 203-936-6677.
